Veritas Sports Performance — Release of Liability & Assumption of Risk

1. Voluntary Participation  

I understand that participation in Veritas Sports Performance training is voluntary. I choose to participate, or authorize the minor participant to participate, with an understanding of the physical demands of athletic training.

2. Assumption of Risk  

I understand that athletic performance training may include sprinting, acceleration, deceleration, jumping, agility, running, strength training, conditioning, mobility work, and other physical activities. These activities involve inherent and foreseeable risks, including falls, collisions, muscle strains, sprains, joint injuries, fractures, overexertion, and other injury or illness. I knowingly and voluntarily assume these risks, including those that cannot be eliminated without changing the nature of the activity.

3. Release of Liability  

To the fullest extent permitted by California law, I release and agree not to hold Veritas Sports Performance, Coach Rick Foley, and their coaches, instructors, employees, agents, and representatives responsible for claims for bodily injury, illness, property damage, or other loss arising from or related to my participation, including claims based on ordinary negligence, except where such a release is prohibited by law. This release does not apply to gross negligence, reckless or intentional misconduct, or any liability that cannot legally be released.

4. Medical Fitness & Disclosure  

I represent that the participant is physically able to participate and that I have disclosed any known medical condition, injury, restriction, or circumstance that could affect safe participation. I understand that Veritas Sports Performance does not provide medical diagnosis or treatment.

5. Emergency Medical Consent  

In the event of injury or medical emergency, I authorize Veritas Sports Performance personnel to seek or arrange reasonable emergency medical assistance when I or the parent/guardian cannot be reached promptly. I understand that any medical or emergency expenses are my responsibility.

6. Parent/Guardian Consent for Minors  

If the participant is under 18, I represent that I am the parent or legal guardian and have authority to enter this agreement on the participant’s behalf. I have read and understand this agreement and authorize the minor to participate subject to its terms.
